Application for registration with the option of the secure card

All first time applications require the following:

  • One Completed Application

  • Original long form birth certificate (birth document showing the parents)

  • Photocopy of front and back of one valid government photo identification of parent/guardian (for 15 and under), or the applicant (if 16 and older).

  • One guarantor form completed by the applicant and guarantor

  • Guarantor to also sign off copy of ID

  • Copy of Court Documents if applicable​

If the Secure Certificate of Indian Status Card is also being requested:

  • Two passport photos, one signed and dated by the Guarantor should also be included in the package.
